The first step of treatment is a detailed intraoral examination, a panoramic X-ray, and, where indicated, a 3D cone-beam CT (CBCT) scan. These images make it possible to measure the height, width, and density of the jawbone with millimetric precision. The position of adjacent teeth, nerve canals, and sinus cavities is also assessed to minimise surgical risk. Your general health, medication list, and any systemic conditions are reviewed in detail at this stage.

Digital Treatment Planning

The examination and imaging data are evaluated together to build a personalised treatment plan. The plan defines the number of implants, brand and model, the ideal position and angulation of each fixture in the bone, the need for bone grafting or a sinus lift, the type of restoration, and the estimated treatment time. At Doredent, digital planning tools are used to simulate implant placement on screen, making the outcome more predictable before surgery begins.

Surgical Implant Placement

Once the treatment plan is approved, implant surgery is performed under local anaesthesia. The gum is opened and an osteotomy site is prepared in the jawbone with dedicated drills, and the titanium fixture is placed. Placing a single implant takes roughly 10–15 minutes. In suitable cases, a temporary tooth (immediate loading) can be fitted in the same session. The procedure is pain-free under anaesthesia; mild post-operative swelling and tenderness are normal and well controlled with prescribed medication.

Osseointegration (Fusion with the Bone)

After placement, the titanium fixture needs time to biologically fuse with the surrounding bone — a process called osseointegration. Depending on bone quality, general health, and the surface technology of the implant, this typically takes between 1 and 4 months. In cases involving bone grafting or sinus lifting, this period can extend to 4–6 months. Throughout this time, the implant sits beneath the gum and integrates with the bone.

Healing Abutment and Gum Shaping

Once osseointegration is complete, a healing abutment is attached to the implant. The abutment shapes the surrounding gum into a natural, aesthetic emergence profile. It typically stays in place for 2–3 weeks, during which a healthy soft tissue cuff forms around the implant. This step is critical for both the aesthetic and functional success of the final restoration.

Final Restoration

Once gum shaping is complete, the final crown is fitted to the implant. A digital impression is taken and a custom restoration is fabricated in the laboratory to match the patient’s anatomy and the shade and shape of the neighbouring teeth. Need for Bone Grafting or a Sinus Lift

To place an implant, the jawbone must have adequate height, width, and density. In All-on-4 treatment, 4 implants are placed per jaw and a fixed full-arch prosthesis is mounted on top. The posterior implants are angled to reduce the need for bone grafting, and temporary teeth can often be fitted the same day. All-on-6 uses 6 implants — the additional anchorage points provide more even load distribution and higher long-term stability. For the first 24 hours after surgery, apply cold compresses to the site, avoid hot food and drinks, and do not smoke or drink alcohol. Stick to soft foods for the first few days and avoid spitting or rinsing. Take prescribed antibiotics and pain relief on schedule. Throughout healing and beyond, pay close attention to oral hygiene and keep to your follow-up schedule.
