Ideally, the upper jaw should be larger than the lower jaw’s arch. The upper teeth are expected to surround the lower teeth completely. However, in cases such as mouth breathing, genetic factors, or early loss of milk teeth or permanent teeth, the development of the upper jaw may be insufficient. When this condition is severe, it can negatively affect the facial profile and appearance, leading to a recessed upper jaw or a long face appearance.

In growing children, the upper jaw bone can be easily expanded with appliances. In adults, the hardened bone does not allow for such expansion. Therefore, bones that prevent expansion can be weakened with surgical cuts to achieve the desired expansion.

Surgical-Assisted Rapid Palatal Expansion (SARPE)

Surgical-assisted rapid palatal expansion (SARPE) can be performed under local anesthesia. However, to increase patient comfort, it is usually done in a hospital setting with general anesthesia or sedation. This procedure is typically carried out by oral and maxillofacial surgeons.

The decision for SARPE is made by orthodontists based on clinical and radiological evaluations. Before the surgery, measurements are taken from the patient, and a custom palatal expander is designed. This appliance is placed in the patient’s mouth before the surgery, and after the operation, the activation protocols for the device begin. The patient or a close relative is instructed on how and how often to activate the device. It is crucial to follow the doctor’s recommendations precisely to achieve the desired results.

Benefits and Procedure

Surgical-assisted rapid palatal expansion can have a positive effect on nasal airflow and breathing. Studies suggest that it improves sleep quality and reduces the need for daytime naps. While research on these topics continues, the proven benefits are mainly orthodontic, such as effectively expanding the upper jaw, eliminating crossbite problems, and supporting the proper alignment of teeth.

SARPE is a well-established method with effective and successful outcomes. Studies show a very low rate of complications. To minimize the risk of complications, the treatment should be meticulously planned by a specialist, and the surgery should be performed carefully by experienced professionals.

I was born in Edirne in 1988. In 2007, I began my master’s degree at Istanbul University (Çapa) Faculty of Dentistry. I graduated in 2012.

In 2014, I started my specialty training in the Department of Orthodontics at Süleyman Demirel University Faculty of Dentistry. I received the title of Orthodontist in 2019 and was appointed to Çorlu ADSM, where I worked as an Orthodontist from 2019 to 2022.

Since 2024, I have been continuing my practice in my own clinic.
